What is home health?

Home health care helps people with specific needs maintain there independence and continue to live at home. This includes all ages! The support comes in all shapes and sizes. First, you have to figure out if you are in need of medical or non-medical support.

Non-Medical Support

  • Companionship

  • Personal care (examples: feeding, oral/personal hygiene, bathing, ambulation, toileting, dressing/grooming)

  • Assistance with Activities of Daily Living

  • Meal preparation

  • Mobility assistance

  • Medication reminders

Medical Support

  • Medication management

  • Hospice Discharge

  • Chronic Care Management (support with long-term illnesses that last at least a year and often get worse over time).
